Preferably include from build dir (#13516)
* Include from build dir first

This fixes out of tree builds by ensuring that configure artifacts are included
from the build dir.

Before, out of tree builds would preferably include files from the src dir, as
the include path was defined as follows (ignoring includes from ext/ and sapi/) :

    -I$(top_builddir)/main
    -I$(top_srcdir)
    -I$(top_builddir)/TSRM
    -I$(top_builddir)/Zend
    -I$(top_srcdir)/main
    -I$(top_srcdir)/Zend
    -I$(top_srcdir)/TSRM
    -I$(top_builddir)/

As a result, an out of tree build would include configure artifacts such as
`main/php_config.h` from the src dir.

After this change, the include path is defined as follows:

    -I$(top_builddir)/main
    -I$(top_builddir)
    -I$(top_srcdir)/main
    -I$(top_srcdir)
    -I$(top_builddir)/TSRM
    -I$(top_builddir)/Zend
    -I$(top_srcdir)/Zend
    -I$(top_srcdir)/TSRM

* Fix extension include path for out of tree builds

* Include config.h with the brackets form

`#include "config.h"` searches in the directory containing the including-file
before any other include path. This can include the wrong config.h when building
out of tree and a config.h exists in the source tree.

Using `#include <config.h>` uses exclusively the include path, and gives
priority to the build dir.

Simplify PDO include paths (#14444)
PDO include paths can be simplified and synced as done in other
extensions: either the project root directory or the phpincludedir (for
the system installation). The 'ext' include is automatically appended
when doing phpize build. In php-src it is only present on Windows build.
The PHP_CHECK_PDO_INCLUDES is left intact working as before and checks
if PDO headers are found.

ce668c0ec6 PGSQL and POD_SQL: don't include pg_config.h
Even if that header file is available, we better consider it private,
and don't include it. The information about whether SSL support is
enabled is now missing (`USE_(OPEN)SSL`), and it seems there is no
alternative way to get it (`PQinitSSL()` is always defined), so we
remove it from the PHP info. Furthermore, the `PG_VERSION` and
`PG_VERSION_STR` macros are no longer available, but as of libpq 9.1
there is `PQlibVersion()` which allows us to construct `PG_VERSION` in
a most likely backwards compatible manner. The additional information
available through `PG_VERSION_STR` is lost, though, so we define
`PGSQL_LIBPQ_VERSION_STR` basically as alias of `PGSQL_LIBPQ_VERSION`,
and deprecate it right away.

Since we are now requiring at least libpq 9.1, we can remove some
further compatibility code and additional checks.

Regarding the raised requirements: official support for PostGreSQL 9.0
ended on 2015-10-08, and even CentOS 7 already has PostGreSQL 9.2, so
this is not supposed to be too much of an issue.

67f9b0b754 Fix #79532: sizeof off_t can be wrong
We have to actually determine the proper `SIZEOF_OFF_T`.
Interestingly, it is `4` on Windows x64.

We also have to prevent the redefinition in pg_config.h. The clean
solution would likely be to not include pg_config.h at all, but that's
out of scope for BC reasons for now.

69b20f51e1 Disable PACKAGE_* preprocessor symbols
Autoconf defines PACKAGE_* symbols:
- PACKAGE_NAME
- PACKAGE_VERSION
- PACKAGE_TARNAME
- PACKAGE_STRING
- PACKAGE_BUGREPORT
- PACKAGE_URL

and appends them to the generated config.h.in files. With AC_INIT change
via afd52f9d99 where package version, URL,
bug report location and similar meta data are defined, these
preprocessor macros are then non empty strings in the generated
configuration header file. When using phpize, PHP shares the config
files in extensions, warnings of redefined macros appear, such as:
- `warning: 'PACKAGE_NAME' macro redefined`

This patch now disables these non utilized symbols in the generated
config header files.

Better practice would be to include only API specific headers where
needed but this would require even more refactorings. Some extensions
such as pcre, pgsql, and pdo_pgsql solve this issue by undefining some
of these symbols before including the library configuration headers in
the code also. Because these symbols can be defined by any library which
uses Autotools.

Additionally, the unused PACKAGE_* symbols were cleaned for the bundled
libmbfl library and with this patch not needed undef code removed.

92ac598aab Remove local variables
This patch removes the so called local variables defined per
file basis for certain editors to properly show tab width, and
similar settings. These are mainly used by Vim and Emacs editors
yet with recent changes the once working definitions don't work
anymore in Vim without custom plugins or additional configuration.
Neither are these settings synced across the PHP code base.

A simpler and better approach is EditorConfig and fixing code
using some code style fixing tools in the future instead.

This patch also removes the so called modelines for Vim. Modelines
allow Vim editor specifically to set some editor configuration such as
syntax highlighting, indentation style and tab width to be set in the
first line or the last 5 lines per file basis. Since the php test
files have syntax highlighting already set in most editors properly and
EditorConfig takes care of the indentation settings, this patch removes
these as well for the Vim 6.0 and newer versions.

With the removal of local variables for certain editors such as
Emacs and Vim, the footer is also probably not needed anymore when
creating extensions using ext_skel.php script.

Additionally, Vim modelines for setting php syntax and some editor
settings has been removed from some *.phpt files. All these are
mostly not relevant for phpt files neither work properly in the
middle of the file.
