Fixes [Bug #16332]
Constant access was changed to no longer allow top-level constant access
through `nil`, but `defined?` wasn't changed at the same time to stay
consistent.
Use a separate defined type to distinguish between a constant
referenced from the current lexical scope and one referenced from
another namespace.
This was an intentional bug added in 1.9.
The approach taken here is to add a second operand to the
getconstant instruction for whether nil should be allowed and
treated as current scope.

set default internal encoding to the excpected message, which
affects String#inspect in messages.
* test/lib/test/unit/assertions.rb (assert_warning): ditto.

tests could not be translates... search '!!' mark to see it.
* test/csv/test_csv.rb: should require 'csv', not '../lib/csv'. test runner
should set load path correctly.
